Strategies for Diabetes Management in the Primary Care Setting

This workshop is designed to help your practice implement the NH Guidelines for Diabetes Care. This two-part program emphasizes the team approach to diabetes care. Held at your practice, this program can be individualized to fit your needs.
Part I - Diabetes Management Update
This one-hour session provides an overview of the NH Guidelines for Diabetes Care, information on new medications and recommendations for early detection and treatment of diabetes complications in adults.
Part II - Diabetes Care ConsultationA diabetes educator leads a review of your current diabetes care process and assists in the identification of strategies to enhance implementation of the guidelines in your practice.

No charge to northern New Hampshire sites. This program is supported by NH AHEC Centers, NH Department of Health and Human Services - Diabetes Education Program, New Hampshire Association of Diabetes Educators, and the Northeast Quality Health Care Foundation.

Northern NH AHEC provides assistance to participants interested in ordering full-text articles using Loansome Doc. This program can be tailored to large groups, small groups, or individuals.

Internet Essentials For Health Professionals: finding & evaluating useful information
The World Wide Web has a wealth of useful, authoritative information fo health professionals. Participants will have the opportunity to gain the essential tools needed to easily locate and evaluate health information on the Internet.

Domestic Violence: a primary health care issue
*This two-hour program can be scheduled on-site at your hospital, practice, agency, school or organization on a date and time that meet your needs.
Participants will have the opportunity to gain the essential tools needed to recognize signs and symptoms of domestic violence and sexual assault as they may present in different populations in the primary care setting. Practitioners will also learn of the referral tools available to them to assist their clients. Practitioners will learn how to integrate screening for domestic violence and sexual assault into their practice.

The World Wide Web for Health Professionals: a 2-hour hands on workshop
Participants learn to perform effective World Wide Web (WWW) searches using a variety of specific Internet tools, locate health information on the WWW quickly, and evaluate WWW sites based on six major criteria. AHEC staff are available during the session to give assistance and answer specific questions.

Web Research for Health Professionals: Clinical Practice Guidelines & Standards of Care
This hands-on course will give attendees a variety of tools to find the most current and authoritative guidelines, protocols and standards of care. Participants will learn how to formulate questions that will lead to appropriate search strategies, and apply search/retrieval techniques in their area of expertise. Workshop can be scheduled at your facility’s computer lab at your convenience, and is offered in the 4 northern counties of Grafton, Coos, Belknap, and Carroll.

Participants will gain an understanding of the health literacy problem, identify barriers in the health care system that limit patients with low literacy, learn how to improve patient-provider communication, and learn strategies to create a respectful patient learning environment.
